Register with the HPCSA

BASA assist only members with HPCSA registration. Non members have to apply directly to HPCSA.

If you are a BASA Member then registration with the Health Practitioners Council of South Africa is done by submitting the relevant forms and info to BASA, who will ensure that the documentation is correct and will deal directly with the HPCSA to ensure registration runs smoothly.

FIRST TIME REGISTRATION WITH HPCSA

STUDENTS HPCSA REGISTRATION PROCEDURES AND CHECKLIST – TO OBTAIN A BKS NUMBER

  1.  HPCSA Form 87 BKS – ORIGINAL – Go to HPCSA to Download Form.
  2. R156.00 HPCSA registration fee for 2013 (1 April 2012 - 31 March 2013) (to be paid directly into HPCSA bank account – see details at the bottom of the page highlighted in red)
  3. Copy of your Proof of Payment (deposit slip).
  4. Certified Copy of ID Document (Drivers License or passport not accepted)
  5. NOTARY MARKED Copy of Pre graduate degree OR Form 23

Please note a NOTARY MARKED copy of your degree means been seen by a NOTARY PUBLIC – this is an attorney who is also a NOTARY – certified or commissioner of oath is NOT accepted as it is not the same. THE STAMP HAS TO PHYSICALLY SAY NOTARY PUBLIC OF SOUTH AFRICA

ALL the above documents will be returned to you if incomplete or you will be notified by E-mail.

INTERN REGISTRATION WITH PROCEDURE AND CHECKLIST TO REGISTER WITH HPCSA

  1. Form 26 BKINPLEASE NOTE - Section C : Two signatures are required from the university and the university stamp  OR  Notary copy of your degree.
  2. HPCSA BK S certificate – copy
  3. Registration fee R170 registration fee for 2013 (1 April 2012 - 1 March 2013) (to be paid directly into HPCSA bank account – see details at the bottom of the page highlighted in red)
  4. Copy of your proof of payment slip.
  5. Copy of your ID document.
  6. A letter form the supervising Bio, stating that he/she is willing to act as supervisor for the intern and stating the period of internship involved. **
  7. A letter from the Head of the Accredited Private practice or Training institution  indicating that the intern will be accommodated for the full duration of the internship as well as the exact period of the internship.   **(Point 6 & 7 information requirements c an be done on 1 letter)
  8. A letter form the Head of Department of the supervising university stating that the university will act as a supervising university and that the department will ensure that training is undertaken in accordance with the approve internship programme.  (This is only applicable for interns doing their internship at the university.  (Point 6 & 7 is not applicable in this instance)

The ORIGINAL HPCSA Form 24 - Go to HPCSA to Download Form

  1. Form 24 signed and stamped by commissioner of oaths – end section A
  2. Form 24 – section B signed by medical practitioner
  3. Form 24 – section C signed by a responsible person
  4. Original Intern Evaluation report completed and signed by your supervisor and yourself. Go to HPCSA to Download Form.
  5. R465.00 registration fee for 2013 (1 April 2012 - 1 March 2013) HPCSA – To be paid directly to HPCSA – see below.
  6. Pro rata year fee HPCSA (see table below) – To be paid directly to HPCSA – see below.
  7. Certified Copy of ID Document (Drivers License or passport not accepted)
  8. NOTARY MARKED COPY of Honours degree or Form 23
  9. Certified copy of HPCSA BK S certificate (student/intern registration with HPCSA)
  10. Copy of your Proof of Payment (deposit slip).
